Kindergarten Registration 2026

Dear Parents/Guardians,

Kindergarten Registration for the 2026-2027 school year will be held in March this year.  There are two ways to register your child:

Registration packets will be available for pick up starting March 2, 2026.

1.  On-line Registration will be accepted starting March 1, 2026.  Complete all the forms and email them to: [email protected] 

2.  If in-person registration is needed it will be by appointment only. Please call Donna Meehan at 267-599-2486 to schedule an appointment. If you get voicemail, please leave your name and number and someone will return your call.

Children must be 5 years old BEFORE September 1, 2026, to qualify for Kindergarten.

Registration forms are available below. Please do not obtain registration packets from any other school.
- Completed registration packet:
                * General Registration Form
                * Use of Electronics
                * Health History Form
                * Home Language Survey
                * Housing Questionnaire
                * Publicity Refusal
                * Title 1
                  *  Race Ethnicity
                  *  Custody Information
        - Child’s birth certificate
- Child’s up-to-date Immunization record
- Parent’s Identification (Address needs to match mortgage or lease)
- 4 proofs of residency
         * One must be a copy of the mortgage, lease, deed, or settlement papers
         * Other items can be found on the 3rd page of the General Registration Form

Health Forms
               a. Student Health History Form
               b. 
Private Physician’s Letter
Required for students in kindergarten or first who are entering school for the first time, for students in 6th and 9th grade.  This form will have to be printed and completed by the child's physician. 
               c. 
Private Dental Letter
This form will have to be printed and completed by the child's dentist. 
               d. 
Medication Policy and Form  
Required for students who will need medication administered by the school nurse during the school day. 
                 e. 
School Vaccination Requirements 
The document explains which vaccinations are required by the fifth day of school.  Failure to provide the school with a record of the vaccination may result in the child being excluded from school. Records may be obtained from the child’s primary physician.
