STUDENT REGISTRATION FOR TRANSFERS FROM ANOTHER SCHOOL.
(Grades 1 through 5)

Registration forms are available below.  They can also be obtained at the Keystone Office between the hours of 8:30 AM to 12:30 PM and 1:30 PM to 3:30 PM. Please do not obtain registration packets from any other school or from any other website link then this one. 

The following is needed at the time of registration:

                * Use of Electronics
                * Custody Information
                * Exchange of Information
                * Health History Form
                * Home Language Survey
                * Housing Questionnaire
                * Prior Discipline Record
                * Private Dentist Letter/Form
                * Private Physician Letter/Form
                * Publicity Refusal
                * Race Ethnicity
                * Title 1
        - Child’s birth certificate
- Child’s up-to-date Immunization record
- Parent’s Identification
- 4 proofs of residency (See first page of packet for acceptable types)
